High Peaks Hospice & Palliative Care Invites Public to Community Connections Day
April 5, 2011 – Queensbury, NY – High Peaks Hospice & Palliative Care invites the public to come out for Community Connections Day at Crandall Park on Saturday, May 7th from 10am till 2pm.
The local InterAgency consortium of non-profits has partnered with the Glens Falls Rotary and it’s annual Water Walk and Fishing Derby in Crandall Park on Saturday, May 7th to
offer the public a unique opportunity to network with human service agencies and local businesses.

InterAgency member stated, “This is a wonderful opportunity for our community, the
Rotary has been very gracious in supporting our goal of re-vamping InterAgency Day and showcasing our organizations.” There will be free health screenings, prizes, giveaways, raffles, activities for all ages, food vendors, and free educational workshops. High Peaks Hospice & Palliative Care, Inc. provides a holistic approach to end-of-life care. Their team of professionals include; nurses (RNs and LPNs), social workers, home health aides, a hospice physician, a bereavement counselor, trained volunteers, chaplains, nutrition counselors and therapists. Hospice is appropriate for any individual, no matter their age, who has been diagnosed with any life limiting illness. Services are covered by the Medicare Hospice benefit as well as private insurance. Contact with hospice should be made as soon after terminal diagnosis as possible.
